The Family Connects registered nurse is a unique nursing opportunity providing newborn/postpartum education to parents in their home. In addition to completing nursing assessments, the Family Connects Nurse provides education and resources on a wide variety of newborn and postpartum topics. Interacting with families shortly after the birth of their child, the Family Connects Nurse travels within Peoria County to complete daily visits. The number of visits each family receives is based upon needs; however the program is designed to be a short term service provided to all new parents regardless of income or risk factors. The successful candidate works independently and with team members, has an upbeat and positive personality and can easily engage and communicate with a wide range of families within the community.
Provides home visiting services to all families living in Peoria County. The goal is to offer support and assess family needs in 12 areas of physical and emotional well-being near the time of birth and to link families with needed, more intensive services using the evidenced based Family Connects model.
- The position requires a current Illinois RN license. - Maternal/newborn nursing experience preferred. - Bilingual in English/Spanish is preferred. - IBCLC or Certified Lactation Specialist is preferred. - Current CPR certification is required. - A vehicle and valid IL driver’s license to transport self to home visits and meetings is required. - Ability to use laptop for data entry. - Ability to conduct in-service training and to mentor new staff in the Family Connects model - Understanding of basic software programs such as Word or Excel - Ability to transport self to meetings and other program activities - Ability to lift 50 pounds - Mental and physical ability to handle the requirements of the position
